Privacy Policy

This policy explains how Functional Medical Institute ("we", "us") collects and uses information through our online store and patient portal at functionaldr.store, our payment and proposal pages, and our email and text messages.

We are a medical practice and a covered entity under HIPAA. Health information we hold as your provider is protected under our Notice of Privacy Practices, which you received at intake and which you can request at any time. Where that notice and this policy differ, the Notice of Privacy Practices governs your health information.

• Account details: name, email address, mobile number, date of birth and shipping address.
• Orders and payments: products ordered, program enrollments, amounts, and payment status. Card and bank details are entered on our payment processors' secure forms; we keep only the card type and last four digits.
• Communications: messages you send us and your email and text preferences.
• Technical information: pages visited, device and browser type, and IP address, used to run and secure the site and to record agreements you sign online.

• To provide your care, programs and orders, and to ship products to you.
• To process payments, send receipts and notify you about payment problems.
• To send account messages such as sign-in codes, order updates and reminders.
• To send offers to patients who have agreed to receive them. You can unsubscribe at any time.
• To keep the site secure and meet our legal obligations.

We do not sell your personal information. We share it only with service providers that help us run the practice (for example payment processing, shipping, hosting, and email and text delivery), under agreements that limit their use of it to providing those services and, where health information is involved, under HIPAA business associate agreements. We also disclose information when the law requires it.

If you agree to receive text messages, we use your mobile number to send the messages described in our Text Message Terms. Message frequency varies and message and data rates may apply. Reply STOP to opt out or HELP for help.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-message opt-in data and consent are not shared with any third parties, except the providers that deliver our messages on our behalf.

Account and transaction emails (receipts, payment notices, sign-in links) are part of our service. Marketing emails include an unsubscribe link; unsubscribing stops marketing email only.

Information is encrypted in transit and at rest, and access is limited to staff who need it. We keep records for as long as needed to provide our services and as medical-records and tax law require.

You can update your details and your email and text preferences in your account, or ask us to access, correct or delete information we hold about you, subject to our legal duty to keep medical and financial records.
